Notice Regarding Filing Claims Due to Damage Caused by Hurricane Sandy:
Those affected by Hurricane Sandy, including homeowners, renters, and businesses, should contact their private insurance carriers; and, they should also contact FEMA at 1-800-621-3362, or visit disasterassistance.gov to register for federal disaster assistance.



 

Personal Injury and Property Damage claims can now be filed electronically via eClaim. The first step is to download the type of claim form you wish to file from the list below. Follow the directions within the form. The more information you can provide, the easier it will be to investigate your claim.

Please note that you must use the Adobe Reader to fill out the form. This is especially true for Macintosh users. Claim forms not completed using the Adobe Reader will not be accepted
